How To Order
Please review the information below, then click on a button at the bottom of the page to choose the Mail Order Form or Online Order Form. Type or print clearly your name, street address, city, state, zip code, and telephone number. For online orders, also include your email address. Please include the year and body style of your car or truck. Some parts may require other information, such as engine type. If this is noted in the catalog page for your part, please include this information as well. Order by description and price of each item.
Body & Fender Orders
Please state year and body type when ordering bodies and fenders.
Payment Terms
Please pay TOTAL AMOUNT DUE by Money Order or Cashier's Check. Personal checks are acceptable, but they must clear the bank before shipment. Make payable to POLI-FORM INDUSTRIES. Please allow 2-4 weeks for delivery on fenders and small parts.
Mastercard or VISA may be used for orders by mail, phone, or online. C.O.D. orders accepted with a
50% deposit.
Payment for all orders must be in U.S. FUNDS ONLY. California residents add 8.0% sales tax.
An 50% deposit is required at the time an order is placed for car bodies.
Prices subject to change without notice. Shipping
Shipping will be made by the most economical means, unless otherwise specified by the purchaser. All orders are shipped freight collect, F.O.B. Watsonville, CA. Call for quotes on truck shipments and bodies.
Crating charges:

Claims
Freight: Claims for damage must be filed with the freight company.
Truck: Claims for damaged good MUST BE MADE WITHIN 5 DAYS OF RECEIPT. Notify the carrier immediately that merchandise they delivered was damaged. Call them for instructions.
United Parcel Service: Merchandise must be held in the original shipping carton for UPS inspection. There is a UPS depot near you. Call them for an inspection.
Post Office: Take entire carton and damaged goods to your local post office. They will help you fill out a claim.
Shortage: Claims for shortage must be made to Poli-Form within 10 days of receipt of order.
Defective Parts: Credits and replacements for defective parts and shipping cost shall be determined by Poli-Form on receipt of claim.
Backorders
Parts temporarily out of stock will be shipped as soon as possible. Most items will be shipped within 2-4 weeks, depending upon the availability from our manufacturers. In the event of a backorder, we will send you a Backorder Status Report indicating the parts which are temporarily out of stock. If you feel the need for a status update, please do not hesitate to contact us. Due to the limited production of some items in this catalog, please plan ahead, and order your parts before you need them.
Returned Merchandise
Special and custom orders are final. Formal request must be made before return of any other merchandise. No returns will be accepted without prior written consent of Poli-Form Industries. Your sales slip, and our letter of approval must accompany all returned parts. All approved returns are subject to a 15% restocking charge. No returns accepted after 60 days with the exception of items which have a specific warranty. All returned parts must be shipped freight prepaid. We do not accept collect shipments. Returned parts must not be scratched, drilled, cut, or mishandled in any way, and must be packaged in such a way that they are in new condition when they arrive at Poli-Form.
Warranty
The following is made in lieu of any warranty, expressed or implied: All products manufactured by Poli-Form Industries are of the highest quality in materials and craftsmanship. The manufacturer or seller has no control over the end use of the products, and is not responsible for, or liable for, any loss or injury or damage, direct or consequential, arising out of the use or inabilities of the products. The user shall assume all risk and liability whatsoever in connection therewith. The buyer assumes responsibility for determining the suitability of any product for a specific application.
Guarantee
All Poli-Form products are manufactured with the utmost care. We take pride in the fact that our products are thicker and stronger than most other leading fiberglass manufacturers. All front fenders are reinforced with a hand laminate of 100 strand fiberglass rope along the bead area to avoid cracking.
